Documents, forms, archives, and where to file
Build a route-specific Polish citizenship evidence plan, find civil and archive records, and identify the authority that handles each process.
Should you order every document first?
Short answer: no.
Start with the possible process and the facts it must prove. Otherwise you may pay for records that do not answer the important question.
Build a one-page chronology first.
What should your chronology include?
For every person in the family line, write:
- every name and spelling variant
- birth date and exact historical place
- parents and legal parentage
- marriage, divorce, legitimation, acknowledgment, and adoption dates
- every citizenship and how it began
- every possible loss, permission, deprivation, renunciation, or restoration
- military service and public office
- residence and migration history
- the next child's birth date
Sort events by legal effective date, not certificate issue date.
Then mark each fact as:
- proved by an official record
- supported only by a research lead
- still unknown
Unknown is not no.
Which civil records connect the family?
Start with full records that identify parents and later annotations.
Useful civil evidence includes:
- full birth certificates
- marriage and divorce records
- death certificates
- name-change records
- paternity or acknowledgment records
- legitimation acts
- adoption applications and final orders
- proof of finality and recognition
A civil record proves the event it records. It does not necessarily prove citizenship.
What is an Urząd Stanu Cywilnego?
An Urząd Stanu Cywilnego, or USC, is a Polish civil registry office.
Newer Polish civil records are generally requested through USC. A full copy, called an odpis zupełny, reproduces the original content and later annotations and can be more useful than a shortened extract.
The official USC copy service explains who may request a record, available formats, fees, and issue periods.
As checked on 7 September 2026, the page listed:
- 22 PLN for an abbreviated copy
- 33 PLN for a full copy
- 17 PLN for a non-exempt proxy
These service details can change. Check the current page before ordering.
An official electronic record must remain in its official electronic form. A printout of an electronically signed XML or PDF may not be the official document.
When do records move to the State Archives?
As a general routing rule, birth registers move after 100 years and marriage or death registers after 80 years. Register closing and transfer can affect the actual holding date.
Check both the relevant USC and territorial State Archive when a record is near the boundary.

What is Szukaj w Archiwach?
Szukaj w Archiwach means “Search in Archives.” It is the official discovery portal for Polish State Archives catalogs and many scans. Szukaj w Archiwach
Search by:
- historical and current place name
- parish or religion
- civil registry office
- archive and fonds, meaning the archive collection
- date range
- name variants
A portal index or scan is a lead. It is not automatically the certified copy needed for a citizenship proceeding.
Request the appropriate certified archive copy and record the archive, collection or fonds, unit, page, and retrieval date.
Which records help prove initial Polish citizenship?
Depending on the place and period, useful evidence may include:
- Polish passport or identity card
- citizenship confirmation or nationality-register entry
- consular registration
- permanent-population books
- municipal domicile or swojszczyzna records
- Prussian nationality and residence documents
- Russian municipal or estate registration
- treaty option or verification decisions
- repatriation records
Birthplace, ethnicity, language, religion, and surname are not substitutes for the legal connector.
Read the ancestor-status guide before interpreting partition or borderland records.
Which foreign citizenship records matter?
Ask the foreign authority for the complete file where possible.
Look for:
- citizenship-at-birth evidence
- petition or application
- oath or declaration
- grant or certificate
- exact effective date
- parent-based or automatic acquisition evidence
- residence records
- a scoped no-record response
Also search for Polish permission to acquire or change citizenship, deprivation, renunciation, delivery, publication, and any effect on a spouse or minor child.
A no-record letter does not prove non-occurrence unless the scope, names, periods, offices, and databases are adequate.
What records are needed for military service or public office?
Collect complete records, not one summary letter.
For military service, look for:
- Polish military registration and duty files
- exemptions, deferments, and release
- foreign enlistment or conscription
- active, reserve, guard, or auxiliary status
- service and discharge dates
- coercion evidence
- Polish consent
For public office, collect the legal appointment, sovereign served, oath, duties, dates, and consent.
These records require specialist analysis under the law then in force. Use the military and public-office guide.
Which form matches which process?
Use current official forms. Do not rely on a saved copy from a commercial site.
The Ministry of the Interior and Administration, known as MSWiA, publishes the central citizenship procedure pages.
- Confirmation: use the form linked by the MSWiA confirmation page
- Restoration: use the form linked by the MSWiA restoration page
- Recognition: use the form linked by the MSWiA recognition page
- Presidential grant: use the form linked by the MSWiA presidential-grant page
- Repatriation: start with the current Repatriation Act and route-specific official guidance
Forms are completed in Polish. Check the current attachment and retrieval date because form files may change without a simple edition number.
Which authority handles which process?
The route and residence decide.
- Confirmation: competent voivode, with overseas filing possible through the competent consul
- Restoration: Minister of the Interior and Administration, with overseas filing through a consul
- Recognition: competent voivode based on residence
- Presidential grant: President, lodged through a voivode or consul
- Repatriation: separate authorities under the Repatriation Act
A wojewoda, or voivode, is the regional government authority that decides confirmation and recognition matters.
The consul does not become the deciding voivode simply because the file is lodged abroad.
Do foreign records need translation or authentication?
Often.
Current guidance may require:
- originals or certified copies
- apostille or consular legalization
- Polish translation by a sworn translator or consul
- a full birth certificate naming parents
- civil registration or transcription
- explanation of name and date conflicts
The exact rules depend on the document, treaty, and competent post. Check before paying for translation or authentication.
Do not mail an irreplaceable original until the receiving authority's current instructions require it.
What are the current fees?
Fees depend on the process and filing place.
As checked on 7 September 2026, central MSWiA pages listed these domestic figures:
- confirmation decision: 277 PLN, plus 17 PLN for a non-exempt power of attorney
- restoration: 1,000 PLN
- recognition: 1,000 PLN
- presidential grant through a voivode: 1,669 PLN
The presidential-grant page listed EUR 450 through a consulate. Other consular tariffs use the local post's currency and can differ.
These are dated reference points, not permanent site-wide prices. Check the competent authority and consulate before paying.
Does six months mean the case will finish in six months?
No.
Current central pages state a six-month administrative period for confirmation, restoration, and recognition, with extensions or exclusions.
Waiting for archives, other authorities, consular requests, suspension, applicant-caused delay, and outside causes can be excluded or add time. Passport and civil-registration steps come later.
There is no binding presidential-grant decision period.
Do not treat an administrative period as an end-to-end promise.
What if the authority has already refused the case?
Preserve:
- the complete decision
- reasons
- filing receipt
- every evidence request and response
- envelope or electronic service evidence
- appeal or reconsideration instructions
- the full submitted packet
Confirmation and recognition generally use a 14-day appeal route through the voivode. Restoration guidance describes a 14-day reconsideration request and later administrative-court review.
Do not calculate a live deadline from this article. The actual decision, service date, and procedural rules control.
What should you do next?
Make a two-column list.
In the first column, write each fact the route must prove. In the second, write the USC, archive, foreign authority, voivode, ministry, or consulate likely to hold it.
That gives you a focused evidence plan instead of a pile of unrelated documents.
